Clues Pointing TO This Diagnosis

Gradual, painless peripheral vision loss (missing a car on the right that was there the whole time) — POAG classically produces insidious peripheral field loss that patients do not self-detect until advanced
Absence of pain, redness, or acute symptoms — open-angle glaucoma is asymptomatic ("the silent thief of sight"), unlike acute angle closure
Optic nerve cupping/widening cup-to-disc ratio — progressive enlargement of the optic cup from neuroretinal rim loss is the structural hallmark
Retinal nerve fiber layer thinning noted on exam — diffuse or focal RNFL loss defines glaucomatous optic neuropathy
Intraocular pressure of 24 mmHg (normal ≤21) — elevated IOP is the major modifiable risk factor and supports the diagnosis
Tunneling/constricting peripheral field with a defect on formal testing — reproducible glaucomatous visual field defects confirm functional damage
OCT showing nerve fiber layer thinning — spectral-domain OCT quantifies RNFL loss and is a mainstay for diagnosis and monitoring
Adult onset at age 69 — POAG risk rises with older age
Slow, decades-long progression noticed only late — typical POAG natural history is gradual over years to decades
Response of IOP to a nightly prostaglandin eye drop (24 → 17 mmHg) — IOP lowering is the only proven treatment and confirms a treatable pressure-driven process

Why It's NOT the Similar Diseases

NOT acute angle-closure glaucoma — angle closure causes sudden severe eye pain, a red eye, halos, nausea/vomiting, and blurred vision; Herbert has painless, gradual, silent field loss
NOT normal-tension glaucoma — normal-tension glaucoma has glaucomatous nerve/field damage with IOP always ≤21 mmHg; Herbert's IOP is elevated at 24
NOT ocular hypertension — ocular hypertension is elevated IOP without optic nerve damage or field loss; Herbert already has cupping, RNFL thinning, and a field defect
NOT cataract — cataract causes diffuse blurring/glare and reduced acuity, not a cupped optic disc with peripheral field loss and elevated IOP
NOT a neurologic (e.g., stroke) visual field defect — glaucomatous loss arises from optic disc cupping and does not respect the vertical midline the way a retrochiasmal (homonymous) defect does

Differential Diagnosis List

Acute angle-closure glaucoma — ruled out by absence of pain, redness, halos, and nausea with a chronic painless course
Normal-tension glaucoma — ruled out by elevated IOP of 24 mmHg rather than IOP consistently ≤21
Ocular hypertension — ruled out by presence of optic nerve cupping, RNFL thinning, and a visual field defect
Cataract — ruled out by cupped optic disc, elevated IOP, and peripheral (not central blurring) field loss
Neurologic/retrochiasmal field defect — ruled out by glaucomatous disc cupping and a defect that does not follow the vertical midline

Key Distinguishing Test

Glaucoma is diagnosed by the triad of optic nerve head/RNFL structural damage (assessed by fundus exam and OCT), corresponding reproducible visual field defects on perimetry, and an open anterior chamber angle on gonioscopy.
Tonometry documents IOP but is a weak stand-alone criterion because IOP is often normal in glaucoma—it has high specificity (~0.94) but low sensitivity (~0.48)—whereas spectral-domain OCT of the RNFL and Humphrey visual field testing carry acceptable diagnostic accuracy.

Board Note

POAG is a chronic, painless, usually bilateral (often asymmetric) optic neuropathy and the most common form of glaucoma; it is the leading cause of irreversible blindness worldwide
Risk factors include older age, elevated IOP, family history, and Black, Latinx, or Asian ancestry; elevated IOP is the only modifiable risk factor
Screen high-risk patients through eye care professionals—the USPSTF found insufficient evidence for general population screening but Medicare covers exams for higher-risk groups
Lowering IOP (target typically a 20–50% reduction) is the only proven treatment; prostaglandin analogs (e.g., latanoprost) are first-line because they are the most efficacious, well-tolerated, and once-daily
Selective laser trabeculoplasty is a reasonable first-line alternative or add-on (per the LiGHT trial) and is especially useful when adherence or drop tolerance is a problem; incisional surgery (trabeculectomy, drainage devices) is reserved for inadequate control
Vision already lost is irreversible, so the goal is preserving remaining vision; refer promptly to ophthalmology for diagnosis, target-pressure setting, and lifelong monitoring with IOP, OCT, and visual fields
Topical beta-blockers (e.g., timolol) lower IOP by reducing aqueous production but have systemic absorption—use caution in asthma/COPD, bradycardia, and heart block
