Clues Pointing TO This Diagnosis

Vertigo triggered by rolling over in bed and by head-position changes (sitting up, tilting back) — positional triggering is the hallmark of BPPV
Brief episodes lasting about 30 seconds — BPPV attacks are transient (<1 minute), unlike sustained vestibular syndromes
No hearing loss and no tinnitus — the absence of auditory symptoms argues against Meniere disease and labyrinthitis
No headache between episodes and no continuous baseline symptoms — points to a dynamic positional disorder rather than migraine or a central lesion
Nausea accompanying the spinning — a common autonomic accompaniment of BPPV
Positive right Dix-Hallpike with provoked nystagmus — the criterion-standard confirmatory finding for posterior canal BPPV
Upbeating (vertical) and torsional nystagmus — the classic mixed torsional-upbeat pattern of posterior canal BPPV
Nystagmus lasting about 25 seconds (transient, self-limited) — transient rather than persistent nystagmus supports a peripheral canalithiasis, not a central cause
Complete resolution after two Epley (canalith repositioning) treatments — a strong response to repositioning is characteristic and further supports the diagnosis
Older woman presenting with positional vertigo — BPPV incidence rises with age and is more common in women

Why It's NOT the Similar Diseases

NOT Meniere disease — Meniere causes attacks lasting hours with fluctuating sensorineural hearing loss and tinnitus; Evelyn's episodes last seconds with no auditory symptoms
NOT vestibular neuritis/labyrinthitis — these cause sustained vertigo lasting days that is present at rest, often after a viral prodrome, whereas Evelyn's vertigo is brief and only positionally triggered
NOT central vertigo (stroke/cerebellar lesion) — central causes produce persistent or downbeat nystagmus and additional neurologic deficits; Evelyn's nystagmus is transient, torsional-upbeat, and she has no focal findings
NOT vestibular migraine — this features headache/migrainous symptoms with longer episodes; Evelyn has no headache and purely positional brief spells
NOT orthostatic hypotension — that causes lightheadedness on standing from a lying/sitting position, not true rotational vertigo with torsional nystagmus on Dix-Hallpike

Differential Diagnosis List

Meniere disease — ruled out by absence of hearing loss and tinnitus and by seconds-long (not hours-long) episodes
Vestibular neuritis/labyrinthitis — ruled out by brief positional episodes rather than sustained vertigo at rest
Central vertigo (posterior circulation stroke/cerebellar tumor) — ruled out by transient torsional-upbeat nystagmus and a normal neurologic exam
Vestibular migraine — ruled out by absence of headache and by purely positional brief spells
Orthostatic hypotension — ruled out by true rotatory vertigo with a positive Dix-Hallpike rather than postural lightheadedness

Key Distinguishing Test

The Dix-Hallpike maneuver is the criterion-standard test: turning the head ~45° toward the tested ear and lowering the patient into head-hanging extension provokes, after a brief latency, transient upbeating-torsional nystagmus (with the upper pole of the eyes beating toward the dependent ear) and vertigo that crescendos then resolves within about 60 seconds—confirming posterior canal BPPV.
Persistent or downbeat nystagmus instead suggests a central disorder and warrants further evaluation.

Board Note

Posterior semicircular canal is affected in 85–95% of cases; diagnosis is clinical via Dix-Hallpike, and no imaging is needed for classic triggered, transient positional vertigo
First-line treatment is a canalith repositioning maneuver (Epley or Semont), which is far more effective than medication; success rises with repeat applications in the same session (~80% after one, up to ~90%+ with repetition)
Vestibular suppressants (meclizine, benzodiazepines) are not recommended as primary treatment—they are inferior to repositioning and may only be used short-term for severe nausea/vomiting, with counseling about sedation and fall risk
BPPV commonly recurs; patients can be taught home repositioning exercises, and clinicians should reassess within about 1 month to document resolution
Obtain imaging (MRI) or reconsider the diagnosis for atypical features: absence of nystagmus on provoking maneuvers, persistent/downbeat nystagmus, failure to respond to repositioning, or focal neurologic signs suggesting a central cause
