Clues Pointing TO This Diagnosis

Acute severe sore throat with painful swallowing (odynophagia) — the hallmark presenting symptom of GAS pharyngitis
Fever over 101°F — history of fever is one of the four Centor criteria pointing to strep
Tonsillar exudates (white patches) — tonsillopharyngeal exudate is a suggestive Centor feature
Tender, swollen anterior cervical lymph nodes — tender anterior cervical adenopathy is a Centor criterion
Absence of cough — lack of cough is a Centor criterion and argues against a viral cause
No rhinorrhea and no other coryzal/URI symptoms — absence of viral features raises the likelihood of bacterial GAS
Headache, nausea, and abdominal pain — these systemic symptoms accompany strep pharyngitis, especially in younger patients
School-age/adolescent patient — GAS is most common between ages 5 and 15
High Centor score (fever, exudate, tender nodes, no cough) — a score of 3–4 substantially raises pretest probability and supports testing/treatment
Positive rapid antigen detection test — a positive RADT is definitive because of its high specificity

Why It's NOT the Similar Diseases

NOT viral pharyngitis — viral illness typically features cough, rhinorrhea, conjunctivitis, or hoarseness, all of which Mildred lacks; her absence of cough and coryza plus exudate/fever/tender nodes points to GAS
NOT infectious mononucleosis — mono causes prominent fatigue, posterior cervical (not just anterior) lymphadenopathy, and often splenomegaly, and a positive rapid strep confirms GAS rather than EBV
NOT peritonsillar abscess — an abscess causes muffled "hot potato" voice, trismus, uvular deviation, and unilateral swelling, none of which Mildred has
NOT epiglottitis — epiglottitis produces drooling, respiratory distress, and a toxic appearance, which are absent here
NOT gonococcal or other atypical pharyngitis — these occur in specific epidemiologic settings and would not produce a positive group A strep test

Differential Diagnosis List

Viral pharyngitis — ruled out by absence of cough/coryza and a positive rapid strep test
Infectious mononucleosis — ruled out by positive GAS testing and lack of posterior adenopathy/splenomegaly
Peritonsillar abscess — ruled out by absence of trismus, muffled voice, and uvular deviation
Epiglottitis — ruled out by absence of drooling, stridor, and respiratory distress
Non–group A (C/G) streptococcal or atypical pharyngitis — ruled out by a positive group A strep–specific test

Key Distinguishing Test

Because clinical features alone cannot reliably distinguish GAS from viral pharyngitis, confirm the diagnosis with a throat-swab rapid antigen detection test (RADT); a positive result is definitive given its ≥95% specificity and does not require culture confirmation.
In children and adolescents, a negative RADT should be backed up by throat culture (or a molecular/NAAT assay) because of lower RADT sensitivity, whereas negative tests in adults generally do not require confirmation.

Board Note

Use the modified Centor (McIsaac) criteria—fever, tonsillar exudate, tender anterior cervical nodes, absence of cough, and age—to identify low-probability patients who do not need testing and to guide who should be swabbed; do not test patients with clear viral features (cough, coryza, conjunctivitis, oral ulcers)
Confirm with RADT (or culture); a positive RADT is sufficient to treat, and negative RADT in children/adolescents needs culture backup
First-line treatment is penicillin or amoxicillin; GAS remains uniformly penicillin-susceptible
For penicillin allergy, use a first-generation cephalosporin (if no immediate hypersensitivity) or a macrolide/clindamycin
Treating GAS prevents acute rheumatic fever (which follows in a small percentage of untreated cases 2–4 weeks later) and suppurative complications such as peritonsillar abscess, hastens symptom resolution, and reduces contagiousness; patients are generally noncontagious after 24 hours of appropriate antibiotics
Refer or evaluate urgently for red flags of deep-space infection—drooling, trismus, muffled voice, neck swelling, or respiratory distress (peritonsillar/parapharyngeal abscess, epiglottitis, Lemierre syndrome)
