Clues Pointing TO This Diagnosis

Preceding October cold virus — acute bacterial rhinosinusitis typically develops as a secondary infection after a viral URI obstructs the sinus ostia and impairs mucosal clearance
Symptoms persisting/worsening past 10 days without improvement — persistence ≥10 days beyond onset of upper respiratory symptoms is a defining criterion for presumed ABRS rather than viral
Facial pressure and fullness over the forehead and cheekbones — facial pain/pressure/fullness is a cardinal symptom of acute rhinosinusitis
Thick yellow (purulent) nasal drainage — purulent anterior/posterior drainage is a core diagnostic feature of acute sinusitis
Nasal obstruction with constant blowing and poor drainage — nasal obstruction accompanying purulent drainage completes the symptom criteria
Headache worse with bending over — positional worsening of facial/sinus pain is characteristic of sinus involvement
Maxillary tenderness to palpation/tapping over the cheekbone — localized sinus tenderness supports maxillary sinus involvement
Impaired ciliary clearance from the antecedent virus — viral damage to cilia with mucus stasis is the mechanism enabling bacterial superinfection

Why It's NOT the Similar Diseases

NOT a simple viral URI/viral rhinosinusitis — viral illness peaks by day 2–3 and improves within about a week; Russell's symptoms persisted and worsened beyond 10 days, meeting criteria for presumed bacterial infection
NOT allergic rhinitis — allergic rhinitis causes clear rhinorrhea, sneezing, and itchy/watery eyes with an allergic trigger, not purulent drainage with fever and focal facial tenderness following a cold
NOT migraine or tension headache — a primary headache lacks purulent nasal drainage, nasal obstruction, and sinus tenderness, and does not follow a viral URI in this pattern
NOT dental (odontogenic) infection — a tooth source produces focal dental pain and is considered when sinusitis is unilateral/refractory; Russell's picture followed a URI with bilateral facial pressure and purulent drainage
NOT an orbital or intracranial complication — there is no proptosis, visual change, abnormal eye movements, periorbital swelling, severe headache, or altered mental status, which would signal extra-sinus spread

Allergic rhinitis — ruled out by purulent (not clear) drainage, fever, and facial tenderness without allergic triggers

Orbital/intracranial complication of sinusitis — ruled out by absence of proptosis, visual changes, periorbital swelling, or neurologic findings

Key Distinguishing Test

Acute sinusitis is a clinical diagnosis; no confirmatory test is routinely needed. The diagnosis of presumed ABRS rests on the temporal pattern—purulent nasal drainage with obstruction and facial pain/pressure that persists ≥10 days without improvement or that worsens after initial improvement (double worsening).
The gold standard, rarely performed, is sinus puncture with aspiration of purulent secretions; imaging (CT) cannot distinguish bacterial from viral disease and is reserved for suspected complications or an alternative diagnosis.

Board Note

Most acute rhinosinusitis is viral and self-limited (lasting 7–10 days); fewer than 2% of viral URIs progress to bacterial infection, so antibiotics are overused—reserve them for the ≥10-day persistence, double-worsening, or severe (fever ≥39°C/102°F with purulent drainage and facial pain ≥3 days) patterns
First-line antibiotic when treatment is chosen is amoxicillin (with or without clavulanate); the 2025 AAO-HNS guideline favors amoxicillin alone for previously healthy adults with mild-to-moderate symptoms, reserving amoxicillin-clavulanate for those at risk of resistant organisms (age >65, recent antibiotics/hospitalization, immunocompromise, severe infection)
For penicillin allergy, use doxycycline or a respiratory fluoroquinolone; macrolides and TMP-SMX are not recommended for initial ABRS therapy
Watchful waiting with a "safety-net" (delayed) antibiotic prescription is an appropriate initial strategy for uncomplicated ABRS, revisiting if symptoms worsen or fail to improve within 7 days
Supportive care—saline irrigation, intranasal corticosteroids, and short-course (<3 days) decongestants—can reduce symptoms; oral corticosteroids are not recommended
Do not image uncomplicated cases; obtain contrast CT/MRI and refer urgently to ENT or the ED for suspected complications—orbital cellulitis (proptosis, visual change, ophthalmoplegia, periorbital edema), meningitis/intracranial abscess (severe headache, altered mental status)
