Clues Pointing TO This Diagnosis

Acute atraumatic monoarticular knee that is hot, swollen, and severely painful — the classic triad of joint pain, swelling, and fever should prompt immediate consideration of septic arthritis
Sudden change from her baseline chronic arthritis to a dramatically inflamed joint overnight — acute worsening of a single joint is a red flag for superimposed infection
Fever accompanying the joint findings — more than half of septic arthritis patients report joint swelling, pain, and fever
Inability to bear weight or move the knee (pain with any motion) — a severely painful joint on any range of motion is characteristic
Recent dental procedure a week earlier — transient bacteremia can hematogenously seed a joint, the most common route of infection
Pre-existing joint disease (chronic knee arthritis) — damaged/abnormal joints are a recognized risk factor and portend worse outcomes
Older age (67) — septic arthritis risk rises with age
Cloudy/purulent synovial fluid on arthrocentesis with a markedly elevated WBC count — low-viscosity purulent fluid with high neutrophils is typical
Positive synovial Gram stain — directly demonstrates organisms and guides empiric therapy
Elevated peripheral white blood cell count — a supportive but nonspecific inflammatory marker
Staphylococcus aureus on culture — the most common causative organism in native-joint septic arthritis
The knee as the involved joint — the knee is the most commonly infected native joint

Why It's NOT the Similar Diseases

NOT a gout/crystalline arthritis flare — although both cause a hot swollen joint, the Gram stain is positive and culture grows S. aureus here; crystals would not explain the positive Gram stain (and crystals do not exclude infection anyway)
NOT an osteoarthritis flare — her chronic OA causes a mechanical ache, not fever, purulent synovial fluid, and a positive Gram stain
NOT a rheumatoid/inflammatory arthritis flare — RA is typically symmetric and polyarticular and would not yield purulent fluid with organisms on Gram stain; RA is instead a risk factor for infection
NOT cellulitis alone — cellulitis involves skin/soft tissue with erythema but does not produce a purulent joint effusion with intra-articular organisms and pain on joint motion
NOT gonococcal arthritis — disseminated gonococcal infection occurs in sexually active young adults (often with tenosynovitis, dermatitis, migratory polyarthralgia), not a 67-year-old with post-dental S. aureus monoarthritis

Differential Diagnosis List

Gout/crystalline arthritis — ruled out by positive Gram stain and S. aureus on culture
Osteoarthritis flare — ruled out by fever, purulent synovial fluid, and organisms on Gram stain
Rheumatoid/inflammatory arthritis flare — ruled out by monoarticular purulent effusion with positive Gram stain
Cellulitis — ruled out by presence of an infected joint effusion and pain on joint motion
Gonococcal arthritis — ruled out by age, clinical context, and S. aureus rather than N. gonorrhoeae

Key Distinguishing Test

Arthrocentesis with synovial fluid analysis is required to confirm the diagnosis and must be obtained before antibiotics whenever feasible; send Gram stain, aerobic and anaerobic cultures, and WBC count with differential (plus crystal analysis).
Purulent, low-viscosity fluid with a high neutrophil-predominant WBC count (classically >50,000/mm³, though lower counts do not exclude infection) and a positive Gram stain or culture confirms septic arthritis.

Board Note

Septic arthritis is a medical emergency: cartilage destruction and permanent joint dysfunction can occur if appropriate antibiotics are not started within 24–48 hours, and case-fatality is roughly 11% (higher in polyarticular disease)
S. aureus is the most common pathogen; empiric therapy must cover S. aureus and streptococci, with gram-negative coverage added for older, immunosuppressed, or bacteremic patients—and MRSA coverage (vancomycin) when risk factors are present
Management combines prompt joint drainage (repeated aspiration, arthroscopic washout, or arthrotomy) with systemic antibiotics; about 30% of cases ultimately require surgery, and hip/shoulder infections more often need operative drainage
Oral antibiotics are noninferior to IV once the patient is improving/source-controlled; total duration is typically 2–6 weeks depending on organism (e.g., MRSA usually 3–4 weeks)
Serum ESR, CRP, and WBC support the diagnosis but are nonspecific; a normal synovial WBC or absence of crystals does not exclude infection
Refer urgently to orthopedics for drainage and consider infectious disease consultation; obtain blood cultures during febrile episodes since bacteremia is common
Routine antibiotic prophylaxis before dental procedures is generally NOT recommended for patients with prosthetic hip or knee joints (2024 AAOS/IDSA), and it is not indicated for native joints; decisions in select high-risk prosthetic-joint patients are individualized
