NOT acute alcoholic hepatitis — alcoholic hepatitis is an acute inflammatory syndrome — fever — leukocytosis — rapid onset jaundice — AST to ALT ratio above 2 — tender enlarged liver. Cirrhosis is the chronic end-stage — scarring over years — small nodular liver — portal hypertension.
NOT cardiac ascites from right heart failure — cardiac ascites causes ascites and edema but the liver is enlarged and tender — elevated JVD. George has portal hypertension pattern — small nodular liver — not enlarged from passive congestion. SAAG above 1.1 in both — ascitic protein helps distinguish.
NOT nephrotic syndrome — nephrotic causes massive proteinuria — edema — low albumin — but no liver abnormality — no spider angiomata — no coagulopathy.
NOT hepatocellular carcinoma — HCC can complicate cirrhosis — focal liver mass — markedly elevated AFP. George's presentation is diffuse cirrhotic changes not a focal mass. Surveillance ultrasound screens for HCC.
NOT non-alcoholic fatty liver disease — NAFLD causes fatty infiltration — elevated liver enzymes — metabolic syndrome — but does not typically cause advanced ascites and coagulopathy without progression to NASH cirrhosis.

Key Distinguishing Test

Liver function tests — AST — ALT — bilirubin — albumin — PT/INR — thrombocytopenia. Child-Pugh or MELD score assesses severity and prognosis. Abdominal ultrasound — nodular liver — splenomegaly — ascites — portal vein flow. Paracentesis — ascitic fluid analysis — SAAG above 1.1 confirms portal hypertension with approximately 97% accuracy. Upper endoscopy evaluates for esophageal varices.
