Young tall thin male — classic demographic — primary spontaneous pneumothorax
Sudden onset sharp chest pain — worse with inspiration
Shortness of breath — acute onset
No trauma — spontaneous — subpleural blebs rupture
Decreased or absent breath sounds on affected side
Hyperresonance on percussion affected side
Chest X-ray — white pleural line — loss of lung markings beyond it
Trachea midline — small pneumothorax not tension
Responds to oxygen and observation for small — needle decompression and chest tube for large

Why It's NOT the Similar Diseases

NOT tension pneumothorax — tension pneumothorax has tracheal deviation AWAY from the affected side — absent breath sounds — severe hemodynamic compromise — hypotension — tachycardia — distended neck veins. Thomas has a midline trachea and stable vital signs — not tension.
NOT pulmonary embolism — PE causes sudden dyspnea — pleuritic chest pain — tachycardia — hypoxia — but normal breath sounds bilaterally — no hyperresonance — chest X-ray is often normal.
NOT pericarditis — pericarditis causes sharp positional chest pain — better leaning forward — friction rub — diffuse ECG changes. No breath sound changes — no hyperresonance — no pleural line on X-ray.
NOT pleuritis — pleuritis causes sharp inspiratory pain from pleural inflammation — friction rub — but no absent breath sounds — no hyperresonance — no pleural line.
NOT acute MI — MI causes pressure pain — not sharp inspiratory — no breath sound changes — ECG and troponin abnormal. Thomas is 22 with no cardiac risk factors.

Key Distinguishing Test

Chest X-ray — shows pleural line with absent lung markings beyond it — confirms pneumothorax and estimates size. CT chest — more sensitive — defines size accurately — identifies blebs. Pulse oximetry and vital signs — assess hemodynamic stability and distinguish from tension. Physical exam — hyperresonance and absent breath sounds on affected side.

📝 PRIMARY VERSUS SECONDARY SPONTANEOUS PNEUMOTHORAX: Primary — no underlying lung disease — tall thin young males — subpleural blebs. Secondary — occurs with underlying lung disease — COPD — CF — Marfan syndrome — more dangerous because less respiratory reserve. TENSION PNEUMOTHORAX — air enters pleural space and cannot escape — mediastinum shifts away — compresses heart and great vessels — immediate needle decompression at second intercostal space midclavicular line — do NOT wait for X-ray. Recurrence risk after first spontaneous pneumothorax is 30 to 50% — second episode usually prompts pleurodesis.
