Bloating — early satiety — pelvic pressure — mass effect
Syncope — from acute pain or vagal response to torsion
Enlarged ovary on ultrasound with solid components
Absent or decreased blood flow on Doppler — though PRESENT Doppler flow does NOT rule out torsion
Elevated CA-125 — not specific — elevated with many ovarian conditions
Responds to surgical detorsion and cystectomy

Why It's NOT the Similar Diseases

NOT ectopic pregnancy — ectopic pregnancy requires a positive pregnancy test — rising beta-hCG — empty uterus. Sandra's pregnancy test is negative.
NOT appendicitis — appendicitis causes RIGHT lower quadrant pain — fever — elevated WBC — rebound tenderness. Sandra has LEFT sided pain — no fever — ultrasound shows ovarian pathology.
NOT ovarian cancer — more common in postmenopausal women — vague symptoms — ascites — elevated CA-125. Sandra is 34 — frozen section shows benign — malignancy must always be in the differential.
NOT pelvic inflammatory disease — PID causes bilateral pelvic pain — fever — cervical motion tenderness — vaginal discharge — positive STI testing. Sandra has unilateral pain — no fever.
NOT endometriosis — causes cyclic pelvic pain — dysmenorrhea — dyspareunia — but does not cause syncope — absent Doppler flow — or acute torsion pattern on ultrasound.

Differential Diagnosis List

Ectopic pregnancy — ruled out by negative pregnancy test
Appendicitis — ruled out by left not right sided pain — no fever — ultrasound showing ovarian pathology
Ovarian cancer — ruled out by age — benign frozen section — though must always be considered especially postmenopause
Pelvic inflammatory disease — ruled out by unilateral pain — no fever — no cervical motion tenderness
Endometriosis — ruled out by absent Doppler flow — acute syncope — torsion pattern on ultrasound

Key Distinguishing Test

Pelvic ultrasound with Doppler — gold standard — enlarged ovary — absent or decreased blood flow raises concern. CRITICAL — per ACOG normal Doppler flow can be present in as many as 60% of surgically confirmed torsion cases — normal flow does NOT rule out torsion. Pregnancy test — always first. CA-125 — elevated in many benign and malignant conditions. Surgical exploration — definitive — allows detorsion and frozen section.

📝 OVARIAN TORSION IS A SURGICAL EMERGENCY: Time is ovary — delay causes ischemia and necrosis. CRITICAL TEACHING POINT — per ACOG normal Doppler arterial flow can be present in up to 60% of surgically confirmed torsion cases. ACOG explicitly states Doppler flow alone should not guide clinical decision making — present flow does NOT exclude torsion. Clinical suspicion in the right setting should prompt surgical exploration even with present Doppler. Even dusky-appearing ovaries should be detorsed — only 18 to 20% of necrotic-appearing ovaries are truly necrotic on pathological examination.
