Why It's NOT the Similar Diseases

NOT osteomalacia — osteomalacia is softening of bone from vitamin D deficiency — bone pain — muscle weakness — low calcium — low phosphate — elevated alkaline phosphatase. Osteoporosis has reduced bone density but normal mineralization — different mechanism.
NOT metastatic bone disease — cancer metastases to spine cause vertebral fractures but also have systemic cancer history — elevated calcium — anemia — abnormal protein electrophoresis — focal lesions on bone scan.
NOT multiple myeloma — myeloma causes lytic bone lesions — elevated protein gap — abnormal SPEP — anemia — renal failure. Barbara has none of these.
NOT traumatic fracture — traumatic fractures require significant force — fall — accident. Barbara's fracture occurred with minimal activity — bending to lift — this is the definition of a fragility fracture.
NOT Paget's disease of bone — Paget's causes elevated alkaline phosphatase — enlarged bones — bone pain — hearing loss. DEXA shows different findings — not reduced density in the same pattern.
