Clues Pointing TO This Diagnosis

Distinct multi-day period of elevated mood with markedly increased energy — abnormally elevated mood plus increased activity is the core Criterion A feature of a manic episode.
Decreased need for sleep ("three days without sleeping, energy to spare") — reduced need for sleep while still feeling energetic is a hallmark Criterion B manic symptom, distinct from insomnia.
Pressured, rapid speech ("talking fast") — being more talkative or feeling pressure to keep talking is a classic manic symptom.
Grandiosity ("grandiose," inflated confidence) — inflated self-esteem or grandiosity is a defining Criterion B feature.
Racing thoughts — flight of ideas/subjective racing thoughts is a core manic symptom.
Impulsive spending and risky business behavior ("spending," seven projects at once, midnight client messages) — excessive involvement in activities with high potential for painful consequences (buying sprees, foolish ventures) is characteristic.
Increased but disorganized goal-directed activity ("started seven projects, finished none") — increased goal-directed activity with multiple overlapping new projects is typical of mania.
Recurrent depressive crashes following the highs — bipolar disorder is defined by alternating mood episodes, and most patients with bipolar I also experience major depressive episodes.
Cyclic, recurrent course over years of "ups and downs" — a longitudinal pattern of recurring mood episodes is central to the diagnosis.
Onset in the late 20s — bipolar disorder characteristically emerges in adolescence and early adulthood.

Why It's NOT the Similar Diseases

NOT major depressive disorder — MDD requires the absence of any manic or hypomanic episode; Vivian's discrete week-long high-energy manic periods establish bipolar, since depressive episodes alone look identical in both conditions.
NOT bipolar II disorder — bipolar II requires hypomania without full mania, but Vivian's episodes carry marked impairment and high-risk consequences (a full manic episode), which defines bipolar I.
NOT cyclothymic disorder — cyclothymia involves subsyndromal highs and lows that never meet full episode criteria, and the presence of a full major depressive episode and full mania excludes it.
NOT schizophrenia/schizoaffective disorder — her symptoms occur within mood episodes rather than as sustained psychosis independent of mood, distinguishing a primary mood disorder from a primary psychotic disorder.
NOT substance- or medication-induced mood disorder — a manic episode cannot be attributed to a substance or another medical condition (Criterion D); her recurrent pattern is independent of any such trigger.

Key Distinguishing Test

Bipolar disorder is a clinical diagnosis; there is no confirmatory laboratory test. DSM-5-TR criteria require at least one lifetime manic episode—an abnormally elevated, expansive, or irritable mood with increased energy lasting at least 1 week (or any duration if hospitalization is required) plus at least three Criterion B symptoms (four if mood is only irritable) causing marked impairment—which confirms bipolar I disorder.
A careful history, ideally corroborated by family or collateral informants, is essential because patients usually present during depression, and baseline labs (renal, thyroid) are obtained to guide medication selection rather than to diagnose.

Board Note

Bipolar I requires one lifetime manic episode; depression is the most common reason patients seek care, so always screen for prior manic/hypomanic periods before treating apparent unipolar depression.
Lithium is a first-line mood stabilizer for both acute mania and long-term maintenance, prevents both manic and depressive relapses, and uniquely reduces suicide risk.
Lithium has a narrow therapeutic window (maintenance level ~0.6–0.8 mmol/L) requiring baseline and periodic monitoring of serum lithium, renal function (creatinine), thyroid function (TSH), and calcium; toxicity causes tremor, GI upset, confusion, and ataxia.
Acute mania is treated with a mood stabilizer (lithium or valproate) and/or an atypical antipsychotic (e.g., quetiapine, aripiprazole, risperidone), often in combination for severe episodes; hospitalization may be needed when insight and safety are compromised.
Avoid antidepressant monotherapy in bipolar disorder—it can trigger a switch to mania/mixed states and rapid cycling, particularly in bipolar I; discontinue antidepressants and stimulants during manic symptoms.
Sleep-wake regulation and adherence are central: sleep deprivation can precipitate mania, and lifelong pharmacotherapy plus psychoeducation reduces relapse.
