Why It's NOT the Similar Diseases

NOT schizophreniform disorder — the total illness duration here reaches about six months; schizophreniform disorder is identical in symptoms but lasts at least 1 month and less than 6 months
NOT brief psychotic disorder — that diagnosis requires symptoms lasting more than a day but remitting within 1 month, whereas Chester's disturbance is continuous for roughly six months
NOT schizoaffective disorder or a mood disorder with psychotic features — there is no prominent concurrent manic or major depressive episode driving the psychosis; the delusions and hallucinations dominate independently of mood
NOT substance-induced psychotic disorder — no substance use is described, and the psychosis is sustained rather than tied to intoxication or withdrawal (a urine drug screen should still be obtained to confirm)
NOT psychosis due to another medical condition (e.g., CNS infection, autoimmune/thyroid disease, tumor) — there are no focal neurologic, infectious, or metabolic findings, and this diagnosis is one of exclusion after basic workup

Key Distinguishing Test

There is no biomarker or imaging test for schizophrenia; it is a clinical diagnosis based on DSM-5 criteria — at least two Criterion A symptoms (with at least one being delusions, hallucinations, or disorganized speech) for a significant part of one month, functional decline, and continuous disturbance for at least six months.
Basic laboratory studies, a urine drug screen, and (when clinically indicated) neuroimaging are used to exclude substance-induced and secondary medical causes, not to confirm the diagnosis.

Classic profile: onset in the late teens to mid-twenties (earlier in men), with positive symptoms (hallucinations, delusions, disorganized speech), negative symptoms (flat affect, alogia, avolition, asociality, anhedonia), and functional decline; cannabis use is among the strongest modifiable risk associations
Diagnosis requires ≥6 months of continuous disturbance with ≥1 month of active symptoms; shorter durations define schizophreniform (1–6 months) and brief psychotic disorder (<1 month)
First-line treatment is a second-generation (atypical) antipsychotic, preferred over first-generation agents because of fewer extrapyramidal effects; aripiprazole and risperidone/paliperidone are common first-episode choices
Because atypicals cause weight gain, hyperglycemia, and dyslipidemia, obtain baseline and periodic metabolic monitoring (weight/BMI, waist circumference, fasting glucose or HbA1c, and lipids); risperidone and paliperidone notably raise prolactin
Clozapine is reserved for treatment-resistant schizophrenia (inadequate response to ≥2 adequate antipsychotic trials) and requires regular absolute neutrophil count monitoring for agranulocytosis
Refer first-episode psychosis to a coordinated specialty care program that combines medication with psychoeducation, family involvement, supported employment/education, and case management for the best long-term outcomes
