NOT toxic adenoma — a solitary autonomous nodule causes focal uptake without orbitopathy or positive TRAb; Harriet has diffuse disease with eye findings
NOT subacute/painless thyroiditis — destructive thyroiditis gives a low radioiodine uptake, negative TRAb, and no true hyperfunction; her T3:T4 ratio, positive antibodies, and goiter argue against it
NOT anxiety disorder — anxiety does not suppress TSH, raise free T4, cause proptosis, or produce a goiter
NOT exogenous/factitious thyrotoxicosis — ingestion of thyroid hormone suppresses uptake and does not cause a goiter, positive TRAb, or eye disease

Key Distinguishing Test

Serum TSH is the best initial screen; a suppressed TSH with elevated free T4 confirms overt hyperthyroidism.
Once thyrotoxicosis is confirmed, a positive TSH-receptor antibody (TRAb/TSI) is pathognomonic for Graves disease, and current guidelines recommend measuring TRAb as the first step to distinguish Graves from other causes.
When antibodies are unavailable or the diagnosis is unclear, a radioactive iodine uptake scan showing diffusely increased uptake confirms Graves.

Graves disease is the most common cause of hyperthyroidism and is autoimmune (stimulating TSH-receptor antibodies); it classically features a diffuse goiter and is the only cause associated with thyroid eye disease and pretibial myxedema
Diagnose with a suppressed TSH and elevated free T4/T3, then confirm the cause with TRAb (positive) rather than defaulting to imaging
