Asymmetric involvement (right knee worse than left) — OA is typically asymmetric, unlike the symmetric pattern of rheumatoid arthritis
Absence of systemic/constitutional symptoms — no fever, weight loss, or fatigue points away from inflammatory or infectious causes

Why It's NOT the Similar Diseases

NOT rheumatoid arthritis — RA causes symmetric small-joint involvement, prolonged morning stiffness (>1 hour), warmth/swelling, and systemic symptoms; Frederick has asymmetric large-joint disease with <30-minute stiffness and no warmth
NOT gout or pseudogout (crystal arthritis) — crystalline arthritis presents acutely with a hot, red, swollen joint and elevated synovial WBCs, whereas Frederick's course is chronic without warmth or erythema
NOT septic arthritis — a joint infection causes acute severe pain, fever, warmth, and markedly elevated synovial WBCs; Frederick is afebrile with a chronic, non-inflamed knee
NOT meniscal tear requiring MRI — an isolated meniscal tear typically causes mechanical locking after injury; Frederick has diffuse degenerative pain with radiographic OA and no locking
NOT anserine/prepatellar bursitis — bursitis is extra-articular with focal tenderness and no joint effusion or radiographic joint-space narrowing

Key Distinguishing Test

Knee OA is a clinical diagnosis based on history and exam (activity-related pain, brief morning stiffness, crepitus, bony enlargement); no blood test confirms it.
Weight-bearing (standing) plain radiographs are confirmatory when needed, showing asymmetric joint space narrowing, osteophytes, subchondral sclerosis, and cysts; the combination of osteophytes plus knee pain has a likelihood ratio near 12.
MRI is reserved for atypical features such as suspected meniscal tear, infection, or tumor.

Board Note

Diagnose knee OA clinically in an older adult with activity-related pain and brief (<30 min) morning stiffness; normal radiographs do not exclude OA, and MRI is rarely needed
First-line management is nonpharmacologic and strongly recommended: patient education/self-management, exercise (quadriceps strengthening, low-impact aerobic), and weight loss if overweight (5–10% loss improves pain and function)
Preferred pharmacologic therapy is topical or oral NSAIDs (lowest effective dose, shortest duration, monitor GI/renal/cardiovascular risk); use a COX-2 inhibitor or add gastroprotection in higher-risk patients, and acetaminophen and tramadol are only conditionally recommended with limited efficacy
Intra-articular glucocorticoid injections provide short-term (a few weeks) pain relief and are useful adjuncts; avoid frequent repeated injections, and hyaluronic acid is not recommended for the knee
Refer for total knee arthroplasty when there is moderate-to-severe symptomatic and radiographic OA (near "bone-on-bone") that fails an adequate trial (typically ≥6 months) of conservative therapy, via shared decision-making
Optimize modifiable perioperative risk factors (BMI, glycemic control, nicotine use) before elective arthroplasty; counsel that ~10–34% of patients still have some persistent pain after surgery
