Key Distinguishing Test

MRI is the confirmatory test when needed, showing disc protrusion/extrusion compressing the nerve root, but it is reserved for patients with severe or progressive neurologic deficits, red flags, or symptoms that fail 4–6 weeks of conservative care and who are candidates for injection or surgery—because disc bulges/protrusions are common in asymptomatic people, early routine imaging is discouraged.
