Clues Pointing TO This Diagnosis

Middle-aged man with a diet of frequent red meat, shellfish, and heavy alcohol — high purine intake and alcohol are classic triggers that raise serum urate and precipitate gout, and male sex is a strong risk factor
Sudden onset waking him at 3 a.m. — gout flares characteristically have rapid, often nocturnal onset with escalation of pain over hours
First metatarsophalangeal (big toe) joint involvement — podagra at the first MTP joint is the classic and most common site of an initial gout flare
Intensely red, hot, swollen joint too tender to touch or bear weight — the exquisitely painful, erythematous monoarthritis is the hallmark inflammatory presentation of a gout flare
Preceding heavy meal and alcohol binge the night before — a discrete dietary/alcohol trigger commonly precedes an acute flare
Monosodium urate crystals on joint aspiration that are needle-shaped and negatively birefringent — this is the gold-standard diagnostic finding, 100% specific for gout
Serum urate checked but noted to be "sometimes normal in the flare" — urate can fall into the normal range during an acute flare, so a normal level does not exclude gout and should be rechecked after resolution
Chronic uric acid overproduction/accumulation from purine breakdown — the underlying pathophysiology of monosodium urate deposition in the joint
Rapid response to anti-inflammatory therapy within days — brisk improvement with anti-inflammatory treatment is typical of a gout flare

Why It's NOT the Similar Diseases

NOT septic arthritis — Arthur has no fever, rigors, or systemic toxicity, and the aspirate showed urate crystals with a negative Gram stain/culture, whereas septic arthritis shows positive Gram stain/culture and marked synovial leukocytosis
NOT pseudogout (CPPD) — his crystals were needle-shaped and negatively birefringent at the first MTP joint, not the rhomboidal, positively birefringent crystals of CPPD that typically affect the knee or wrist with chondrocalcinosis
NOT cellulitis — the erythema is centered on and within the joint with a crystal-proven effusion, rather than skin infection with cutaneous erythema spreading distant from the joint
NOT osteoarthritis — his presentation was acute, intensely inflammatory, and monoarticular, unlike the insidious, chronic, minimally inflammatory course of osteoarthritis
NOT rheumatoid arthritis — he has an acute single-joint flare rather than the insidious, chronic, symmetric small-joint polyarthritis with positive rheumatoid factor/anti-CCP seen in rheumatoid disease

Differential Diagnosis List

Septic arthritis — ruled out by absence of fever/systemic toxicity and synovial fluid showing urate crystals with negative Gram stain and culture
Pseudogout (CPPD) — ruled out by needle-shaped, negatively birefringent crystals at the first MTP joint rather than rhomboidal, positively birefringent crystals with chondrocalcinosis
Cellulitis — ruled out by a crystal-positive joint effusion and inflammation localized to the joint
Osteoarthritis — ruled out by the acute, intensely inflammatory monoarticular onset
Rheumatoid arthritis — ruled out by the acute monoarticular flare rather than chronic symmetric small-joint polyarthritis

Key Distinguishing Test

The gold standard is joint aspiration with polarized light microscopy of synovial fluid; identifying needle-shaped, negatively birefringent monosodium urate crystals is essentially 100% specific and confirms gout, while a Gram stain and culture are done at the same time to exclude coexisting septic arthritis.
In classic presentations (e.g., podagra with typical risk factors), a confident clinical diagnosis can be made without aspiration; note that serum urate may be normal during a flare and should be rechecked 2–4 weeks later.

Board Note

Classic associations: middle-aged/older man, purine-rich diet, alcohol (especially beer), obesity, hypertension, chronic kidney disease, and diuretic use; podagra at the first MTP joint is the signature presentation.
Acute flare — first-line therapy is an NSAID, colchicine, or a glucocorticoid (oral or intra-articular), chosen by comorbidities; all have similar efficacy, and low-dose colchicine is preferred over high-dose because of fewer side effects.
Avoid NSAIDs in kidney disease, heart disease, or peptic ulcer disease, and prefer glucocorticoids/colchicine in those settings.
Do not start or stop urate-lowering therapy solely because of a flare; if the patient is already on it, continue it through the flare.
Urate-lowering therapy (ULT) is indicated for recurrent flares (≥2/year), tophi, radiographic joint damage, or urolithiasis; allopurinol is first-line, started low (e.g., 100 mg/day, or 50 mg/day if reduced renal function) and titrated to a serum urate target of less than 6 mg/dL.
When starting ULT, give anti-inflammatory prophylaxis (e.g., low-dose colchicine or an NSAID) for 3–6 months to prevent mobilization flares.
When to refer/escalate: any suspicion of septic arthritis (aspirate urgently), diagnostic uncertainty, refractory or tophaceous disease, or failure to reach urate target despite optimized allopurinol.
