Why It's NOT the Similar Diseases

NOT simple UTI — simple UTI causes local symptoms without systemic involvement. Dorothy has systemic organ dysfunction — confusion — hypotension — tachycardia — elevated lactate — that's sepsis not simple UTI.
NOT septic shock alone — septic shock is a subset of sepsis with hypotension requiring vasopressors despite adequate fluid AND elevated lactate above 2. Dorothy has sepsis that may progress to shock.
NOT delirium from another cause — acute confusion in elderly always requires infection workup — but delirium without hemodynamic instability and fever is not sepsis. Dorothy has both.
NOT meningitis — meningitis causes confusion and fever but also severe headache — neck stiffness — photophobia. Dorothy has a clear urinary source and no meningeal signs.
NOT dehydration alone — dehydration causes confusion and low blood pressure but without fever — tachycardia — elevated inflammatory markers — and an infectious source.

Septic shock — consider if vasopressors required despite fluids
Meningitis — ruled out by urinary source — absence of meningeal signs

Key Distinguishing Test

Blood cultures before antibiotics — identify causative organism. Urine culture confirms source. Lactate — elevated above 2 mmol/L indicates tissue hypoperfusion. qSOFA — respiratory rate above 22 — altered mentation — systolic BP below 100. Sepsis-3 definition — life threatening organ dysfunction from dysregulated host response to infection — SOFA score increase of 2 or more.

Septic shock — vasopressor requirement to maintain MAP above 65 AND lactate above 2 despite adequate resuscitation — mortality above 40%. Blood cultures BEFORE antibiotics — but do NOT delay antibiotics more than 1 hour. Organ dysfunction — not SIRS criteria — defines sepsis. Altered mental status in elderly with infection is a red flag for sepsis.
