Responds to empiric antibiotics — cephalosporins — steroids

Why It's NOT the Similar Diseases

NOT viral meningitis — viral meningitis causes similar symptoms but typically less severe — CSF shows lymphocytes not neutrophils — normal glucose — NO purpuric rash — self-limiting. Brandon's purpuric rash and CSF neutrophilia point to bacterial.
NOT subarachnoid hemorrhage — SAH causes thunderclap headache but no fever — no rash — CT shows blood. Brandon has fever and purpuric rash — infection not hemorrhage.
NOT encephalitis — encephalitis is inflammation of brain PARENCHYMA — seizures — focal deficits — personality changes. Meningitis is inflammation of the LINING — neck stiffness is the hallmark.
NOT tension headache or migraine — neither causes fever — neck stiffness — purpuric rash — or CSF abnormalities.
NOT brain abscess — causes headache — fever — focal deficits — CT shows ring-enhancing lesion. LP contraindicated if abscess suspected without CT first.

Viral meningitis — ruled out by purpuric rash — CSF neutrophilia — low CSF glucose — severity
Subarachnoid hemorrhage — ruled out by fever — purpuric rash — CT shows no blood
Encephalitis — ruled out by meningeal signs predominating — no seizures initially
Brain abscess — ruled out by CT before LP — purpuric rash — meningeal signs
Migraine — ruled out by fever — neck stiffness — purpuric rash — CSF abnormalities

Key Distinguishing Test

Lumbar puncture — CSF analysis — cloudy — elevated opening pressure — neutrophilic pleocytosis — elevated protein — low glucose — Gram stain and culture identify organism. CT head before LP if focal neurological deficits — papilledema — or altered consciousness. DO NOT delay antibiotics for CT or LP if bacterial meningitis suspected.

📝 NON-BLANCHING RASH IS AN EMERGENCY: A non-blanching petechial or purpuric rash with fever and meningeal signs is meningococcemia until proven otherwise — can kill within hours. Give antibiotics IMMEDIATELY — do not wait for LP results. Glass test — pressing a glass against the rash — if it does not blanch it is a true emergency. Close contacts require chemoprophylaxis — single dose ciprofloxacin or a TWO-DAY course of rifampin — 4 doses over 2 days. MCV4 vaccine recommended for all college freshmen.
