Known Type 1 diabetes — insulin dependent
Missed insulin dose — classic trigger
Nausea — vomiting — abdominal pain — classic triad
Kussmaul breathing — deep rapid breathing — blowing off CO2 to compensate for metabolic acidosis
Fruity acetone breath — ketones being exhaled
Severely elevated blood sugar — hyperglycemia
Metabolic acidosis — low pH — low bicarbonate
Elevated anion gap — ketoacids accumulating
Hypokalemia developing with insulin treatment — critical monitoring
Responds to IV fluids — insulin — electrolyte replacement

Why It's NOT the Similar Diseases

NOT hyperosmolar hyperglycemic state — HHS — HHS occurs more commonly in TYPE 2 diabetes — extremely high blood sugar often above 600 — minimal or no ketoacidosis — pH normal or near normal — more profound dehydration. Note DKA can occur in both Type 1 and Type 2 diabetes — but Tyler is Type 1 with significant acidosis and ketones.
NOT alcoholic ketoacidosis — occurs after heavy drinking and poor nutrition — blood sugar is normal or LOW — not markedly elevated like Tyler's.
NOT lactic acidosis — elevated anion gap but from lactate not ketones — typically from shock — sepsis — blood sugar not markedly elevated.
NOT simple hyperglycemia without acidosis — simple hyperglycemia does not cause acidosis — Kussmaul breathing — or fruity breath. Ketosis and acidosis define DKA.
NOT gastroparesis crisis — causes nausea and vomiting in diabetics but no ketoacidosis — no Kussmaul breathing — no fruity breath.

Differential Diagnosis List

HHS — ruled out by significant ketoacidosis — younger age — though overlap can occur
Alcoholic ketoacidosis — ruled out by markedly elevated blood sugar — not low or normal
Lactic acidosis — ruled out by ketones on urinalysis — elevated beta-hydroxybutyrate — not lactate
Sepsis causing hyperglycemia — important to consider as DKA trigger — check for infection source
Starvation ketosis — ruled out by degree of hyperglycemia and acidosis severity

Key Distinguishing Test

Blood glucose — markedly elevated. Arterial blood gas — low pH — low bicarbonate — confirms metabolic acidosis. Anion gap — elevated from ketoacids. Urine or serum ketones — markedly elevated. Beta-hydroxybutyrate — most sensitive ketone marker. Electrolytes — potassium critical to monitor every 2 hours.

📝 POTASSIUM IN DKA: Total body potassium is DEPLETED despite normal or high serum levels — acidosis shifts potassium out of cells. When insulin is given — potassium shifts back INTO cells — serum potassium drops rapidly — can cause fatal arrhythmias. NEVER start insulin without checking potassium. If potassium is below 3.3 to 3.5 — replace potassium BEFORE starting insulin. Monitor every 2 hours during treatment. IV to SC insulin transition — wait until anion gap closes — pH above 7.3 — bicarbonate above 18 — patient tolerating oral intake — overlap insulin by 2 to 4 hours.
