Why It's NOT the Similar Diseases

NOT biliary colic — biliary colic is brief (usually <6 hours), self-limited pain without fever, leukocytosis, or a positive Murphy sign, whereas Rachel's pain was prolonged and unremitting with fever and RUQ tenderness
NOT acute pancreatitis — pancreatitis produces epigastric pain radiating to the back with markedly elevated lipase; Rachel has RUQ pain with a positive Murphy sign and a stone in the gallbladder on ultrasound
NOT choledocholithiasis/acute cholangitis — common bile duct obstruction causes jaundice and elevated liver enzymes/bilirubin (with Charcot triad in cholangitis); Rachel's picture is localized gallbladder inflammation without those features
NOT peptic ulcer disease — PUD causes epigastric burning related to meals without a positive Murphy sign, fever, or the sonographic findings of gallbladder inflammation
NOT acute appendicitis — appendicitis localizes to the right lower quadrant with periumbilical-to-RLQ migration, not RUQ pain with a positive Murphy sign and gallstones on imaging

Key Distinguishing Test

Right upper quadrant ultrasound is the first-line imaging test (sensitivity ~81%, specificity ~83%); findings that confirm acute cholecystitis include gallstones, gallbladder wall thickening/edema, pericholecystic fluid, distention, and a sonographic Murphy sign.
When ultrasound is equivocal but suspicion remains high, hepatobiliary scintigraphy (HIDA scan) is the most accurate confirmatory test (sensitivity ~96%, specificity ~90%); nonvisualization of the gallbladder indicates cystic duct obstruction.

Board Note

Gallstone obstruction of the cystic duct causes 90–95% of cases; about 5–10% are acalculous, typically in critically ill patients
Diagnosis rests on RUQ ultrasound first; reserve HIDA for equivocal ultrasound with high clinical suspicion
First-line treatment is early laparoscopic cholecystectomy, ideally within 1–3 days of diagnosis (and within 7 days of admission/10 days of symptom onset), which improves outcomes over delayed surgery
Give supportive care (IV fluids, bowel rest, analgesia) and perioperative antibiotics; for uncomplicated disease antibiotics are not continued postoperatively
Early laparoscopic cholecystectomy is recommended in pregnancy (any trimester) and in older adults, who have lower mortality with surgery than nonoperative management
For patients at exceptionally high perioperative risk or who are critically ill, percutaneous cholecystostomy (or endoscopic gallbladder drainage) is a bridge or alternative to surgery
Complicated cholecystitis (gangrene, perforation, pericholecystic abscess, cholecystoenteric fistula) requires urgent surgical source control—antibiotics alone are inadequate
