No blood flow on Doppler ultrasound — confirms ischemia — though present flow does NOT rule out torsion
Time critical — must operate within 6 hours for highest salvage rate

Why It's NOT the Similar Diseases

NOT epididymitis — epididymitis causes gradual onset scrotal pain — fever — dysuria — tender indurated epididymis — positive urinalysis or culture — preserved cremasteric reflex — NORMAL or increased blood flow on Doppler.
NOT orchitis — inflammation of the testicle — often from mumps — gradual onset — fever — swollen tender testicle — preserved blood flow on Doppler.
NOT inguinal hernia — inguinal hernia causes groin pain — palpable mass extending to scrotum — bowel sounds in the scrotum — does not cause absent Doppler flow to testicle.
NOT trauma — traumatic injury has a clear history of direct blow — hematocele or fracture on ultrasound — Anthony denies trauma — woke from sleep.
NOT appendix testis torsion — torsion of a small remnant — causes blue dot sign — less severe pain — preserved blood flow — managed conservatively.

Key Distinguishing Test

Doppler ultrasound — absent blood flow supports torsion — but present flow does NOT exclude torsion. Absent cremasteric reflex — most sensitive physical exam finding. Clinical diagnosis in high suspicion — do NOT delay surgery for imaging. Surgical exploration is both diagnostic and therapeutic.
