Why It's NOT the Similar Diseases

NOT COPD — COPD causes OBSTRUCTIVE pattern — low FEV1/FVC ratio — barrel chest — hyperresonance — productive cough — smoking history. Howard has RESTRICTIVE pattern — normal FEV1/FVC — velcro crackles — dry cough — no barrel chest. Obstructive versus restrictive is the fundamental distinction.
NOT heart failure — CHF causes wet crackles — elevated JVD — peripheral edema — cardiomegaly — responds to diuretics. Howard has dry velcro crackles — no edema — no JVD — no cardiomegaly.
NOT pneumonia — pneumonia is acute — fever — productive cough — focal consolidation on X-ray — responds to antibiotics. Howard's course is chronic — over years — dry cough — diffuse basal changes.
NOT sarcoidosis — sarcoidosis causes upper and mid lobe involvement — bilateral hilar lymphadenopathy — multisystem involvement. Howard has basal predominant disease — no hilar adenopathy.
NOT hypersensitivity pneumonitis — upper lobe predominance — caused by organic antigens — bird droppings — mold — improves with antigen avoidance. Howard has basal predominant disease.

COPD — ruled out by restrictive not obstructive pattern and velcro crackles not wheeze
Heart failure — ruled out by dry crackles — no edema — no JVD — no cardiomegaly
Pneumonia — ruled out by chronic course — dry cough — CT pattern
Sarcoidosis — ruled out by basal predominance — no hilar adenopathy
Hypersensitivity pneumonitis — ruled out by inorganic occupational exposure and CT pattern
Connective tissue disease related ILD — ruled out by autoimmune panel

Key Distinguishing Test

High resolution CT chest — honeycombing — traction bronchiectasis — basal and subpleural predominance — UIP pattern. PFTs confirm restrictive pattern. Surgical lung biopsy if CT atypical. Multidisciplinary discussion — pulmonology — radiology — pathology.

📝 IMPORTANT DISTINCTION: Silicosis and IPF are distinct conditions. Silicosis is an occupational pneumoconiosis caused specifically by silica inhalation — upper lobe predominance — egg shell calcifications of lymph nodes — the underlying mechanism is known. IPF — idiopathic pulmonary fibrosis — is by definition of unknown cause — basal predominant — UIP pattern on CT. Howard's story leaves etiology somewhat ambiguous — the clinical features described match IPF/UIP pattern. Students should know these are separate diagnoses even though both can cause pulmonary fibrosis.
