Why It's NOT the Similar Diseases

NOT left heart failure — left heart failure causes pulmonary edema — bilateral wet crackles — reduced left EF on echo — cardiomegaly. Gerald has NORMAL left EF — no pulmonary edema — the right heart is failing BECAUSE of the lungs. Echo is the definitive distinguishing test.
NOT cardiac tamponade — tamponade causes hypotension — muffled heart sounds — elevated JVD — pulsus paradoxus — ACUTE presentation. Gerald's right heart failure is CHRONIC over years — echo shows RV hypertrophy not pericardial effusion.
NOT nephrotic syndrome — causes massive proteinuria — peripheral edema — ascites — but NO elevated JVD — NO right ventricular enlargement — NO pulmonary hypertension. Urinalysis with heavy proteinuria distinguishes it.
NOT primary pulmonary arterial hypertension — PAH occurs in young women without significant lung disease — normal PFTs. Gerald has severe COPD — pulmonary hypertension is SECONDARY to his lung disease.
NOT constrictive pericarditis — causes right heart failure signs but also pericardial thickening on imaging — Kussmaul sign — pericardial knock — no underlying lung disease.

Left heart failure — ruled out by normal left EF on echo — absence of pulmonary edema
Cardiac tamponade — ruled out by chronic course — no muffled heart sounds — echo
Nephrotic syndrome — ruled out by JVD elevation and right ventricular enlargement
Primary pulmonary arterial hypertension — ruled out by severe underlying COPD as cause
Constrictive pericarditis — ruled out by absence of pericardial thickening on echo

Key Distinguishing Test

Echocardiogram — shows right ventricular hypertrophy and dilation — elevated estimated pulmonary artery pressures — NORMAL left ventricular function. This single finding distinguishes cor pulmonale from left heart failure. PFTs confirm severe underlying obstructive lung disease. Oxygen saturation monitoring confirms chronic hypoxemia.

📝 LTOT EVIDENCE: Long-term oxygen therapy — at least 15 hours per day — is the only intervention proven to reduce mortality in COPD patients with chronic hypoxemia — SpO2 below 88% or PaO2 below 55 mmHg. It works by reducing hypoxic pulmonary vasoconstriction — lowering pulmonary vascular resistance — and reducing right heart afterload. The landmark NOTT and MRC trials established this evidence. More oxygen is not necessarily better — a 2024 NEJM trial showed 24 hours per day did not improve outcomes compared to 15 hours.
