WBC ~80,000 with 25% circulating blasts on smear — marked leukocytosis with peripheral blasts strongly suggests acute leukemia

Why It's NOT the Similar Diseases

NOT acute myeloid leukemia — the marrow shows lymphoblasts by flow cytometry rather than myeloid blasts with Auer rods; Auer rods are a morphologic hallmark of AML and essentially absent in ALL
NOT chronic myeloid leukemia — despite carrying the Philadelphia chromosome, his acute marrow failure with ~90% blasts is acute leukemia, not the mature granulocytic proliferation of chronic-phase CML
NOT immune thrombocytopenia (ITP) — isolated low platelets would not explain his anemia, 25% peripheral blasts, and 90% marrow blasts
NOT infectious mononucleosis — atypical lymphocytes and adenopathy from EBV do not produce true lymphoblasts, severe pancytopenia, or a 90%-blast marrow
NOT aplastic anemia — aplasia produces a hypocellular, blast-poor marrow, whereas his marrow is hypercellular and packed with blasts

Differential Diagnosis List

Acute myeloid leukemia — ruled out by lymphoid immunophenotype on flow cytometry and absence of Auer rods
Chronic myeloid leukemia — ruled out by ~90% blasts defining acute leukemia rather than chronic-phase disease
Immune thrombocytopenia — ruled out by concurrent anemia and circulating/marrow blasts
Infectious mononucleosis — ruled out by true blasts and marrow replacement rather than reactive lymphocytosis
Aplastic anemia — ruled out by a hypercellular, blast-rich marrow

Key Distinguishing Test

Bone marrow aspirate and biopsy with flow cytometric immunophenotyping is confirmatory: demonstration of ≥20% lymphoblasts (his marrow shows ~90%) with a B-lineage immunophenotype establishes B-cell ALL. Cytogenetics/FISH and RT-PCR for BCR::ABL1 then identify the Philadelphia chromosome, which defines Ph+ B-ALL and directs the addition of a tyrosine kinase inhibitor.

Board Note

ALL is the most common childhood cancer and also affects adolescents/young adults; the classic tetrad is fatigue (anemia), bruising/bleeding (thrombocytopenia), infection (neutropenia), and bone pain/adenopathy from infiltration.
Diagnosis requires ≥20% marrow lymphoblasts with flow cytometry for lineage; distinguishing ALL from AML rests on immunophenotype, with Auer rods pointing to AML.
Any newly diagnosed acute leukemia warrants urgent referral to a specialized center; ALL treatment proceeds in phases (induction, consolidation, maintenance) with CNS prophylaxis via intrathecal chemotherapy.
Philadelphia chromosome–positive B-ALL is treated by adding a BCR::ABL1 tyrosine kinase inhibitor to chemotherapy/immunotherapy; MRD status guides intensification and transplant decisions.
Tumor lysis syndrome (hyperkalemia, hyperphosphatemia, hyperuricemia, hypocalcemia, AKI) is a key early complication—prevent with aggressive IV hydration plus allopurinol, and use rasburicase for high-risk/high–uric acid patients (check G6PD first).
