Key Distinguishing Test

Erectile dysfunction is largely a clinical diagnosis based on history and validated questionnaires, with organic disease suggested by gradual, consistent, generalized symptoms. Guidelines make a fasting glucose/HbA1c, lipid profile, and morning total (and free) testosterone essentially mandatory in every man newly presenting with ED, both to identify treatable causes and to screen for cardiovascular risk; preserved nocturnal/spontaneous erections point to psychogenic ED, while their loss supports an organic cause.

Board Note

ED is an independent, early marker of cardiovascular disease and typically precedes coronary artery disease, stroke, and peripheral arterial disease by an average of about 3 years (range 2–5), so a new diagnosis warrants cardiovascular risk assessment
First-line therapy for organic ED, including in men with diabetes, is a PDE5 inhibitor (sildenafil, tadalafil, vardenafil, avanafil) combined with lifestyle modification and optimization of glycemic, blood pressure, and lipid control
PDE5 inhibitors are contraindicated with nitrates because of the risk of severe hypotension; avoid nitroglycerin within ~24 hours of sildenafil/vardenafil and ~48 hours of tadalafil
Testosterone replacement is indicated in men with confirmed hypogonadism (two low morning testosterone levels) and can improve erectile function, though the benefit is smaller in men with diabetes and obesity, and combination with a PDE5 inhibitor is often needed for more severe ED
