Clues Pointing TO This Diagnosis

Twenty-year history of diabetes — long-standing diabetes is the leading cause of CKD, and diabetic kidney disease is typically a clinical diagnosis in a patient with long diabetes duration.
Gradual, progressive creatinine rise over 3 years (1.2 → 2.0) — a slow, sustained decline in function over more than 3 months defines chronic (not acute) kidney disease.
Proteinuria/albuminuria on urinalysis — albuminuria without gross hematuria is the characteristic early marker of presumed diabetic nephropathy.
eGFR of 42 mL/min/1.73 m² — a persistent eGFR <60 places her in stage G3 (specifically G3b, 30–44), confirming CKD.
Explicitly "not the acute kind — steady, slow" — chronicity over years distinguishes CKD from acute kidney injury.
Years of hyperglycemia damaging small vessels — microvascular injury from sustained high glucose is the mechanism of diabetic kidney disease.
Hypertension requiring tighter blood pressure targets — hypertension is both a driver and consequence of CKD and clusters with diabetic nephropathy.
Development of anemia with low erythropoietin — anemia from reduced erythropoietin production is a classic complication of progressive CKD.
Bone-mineral concerns with vitamin D and phosphate management — CKD–mineral bone disorder from impaired phosphate excretion and vitamin D activation is expected as GFR falls.

Why It's NOT the Similar Diseases

NOT acute kidney injury — AKI evolves over days, while Sylvia's creatinine climbed slowly over years with a stable, progressive course.
NOT a primary glomerulonephritis — an active sediment (red cells, cellular casts) or rapidly rising proteinuria would suggest nephritis; her picture is bland albuminuria on a background of long-standing diabetes.
NOT nephrotic syndrome from another cause — she lacks the massive proteinuria, edema, and hypoalbuminemia that would prompt biopsy for a distinct glomerular disease.
NOT nephrolithiasis/obstruction — there is no colicky flank pain, hematuria, or hydronephrosis; her decline is a filtration problem, not obstruction.
NOT isolated hypertensive nephrosclerosis alone — although hypertension contributes, her long diabetes duration plus albuminuria makes presumed diabetic kidney disease the primary label.

Differential Diagnosis List

Acute kidney injury — ruled out by the slow rise in creatinine over years rather than days.
Glomerulonephritis — ruled out by the absence of an active urinary sediment or rapidly increasing proteinuria.
Non-diabetic nephrotic syndrome — ruled out by the absence of nephrotic-range proteinuria, edema, and hypoalbuminemia.
Obstructive uropathy/nephrolithiasis — ruled out by the absence of flank pain, hematuria, or obstruction on the clinical picture.
Hypertensive nephrosclerosis as sole cause — ruled out (as primary) by long diabetes duration plus albuminuria pointing to presumed diabetic nephropathy.

Key Distinguishing Test

CKD is a clinical diagnosis confirmed by two measures obtained on a persistent basis (>3 months): an estimated GFR (from serum creatinine) and a spot urine albumin-to-creatinine ratio. A sustained eGFR <60 mL/min/1.73 m² and/or albuminuria ≥30 mg/g confirms and stages CKD using the KDIGO CGA system (Cause, GFR category G1–G5, Albuminuria category A1–A3); Sylvia's eGFR of 42 places her in G3b.
In a patient with long-standing diabetes, bland albuminuria, and no atypical features, this is presumed diabetic nephropathy without need for kidney biopsy.

Board Note

Diabetes and hypertension are the two most common causes of CKD; screen patients with diabetes annually with eGFR and spot UACR.
First-line kidney-protective therapy pairs a RAS inhibitor (ACE inhibitor or ARB, titrated to maximally tolerated dose) with an SGLT2 inhibitor; the SGLT2 inhibitor can be started and continued down to an eGFR of about 20 mL/min/1.73 m². Do not combine an ACE inhibitor with an ARB (risk of hyperkalemia and AKI without added benefit).
Comprehensive care also includes glycemic control (metformin when eGFR ≥30), blood pressure control, statin therapy for most patients ≥50, and lifestyle measures; a nonsteroidal MRA (finerenone) can be added in type 2 diabetes with persistent albuminuria and normal potassium already on standard therapy.
Monitor for and manage complications as GFR falls: anemia (check iron studies; consider iron and refer for erythropoiesis-stimulating agents when hemoglobin <10 g/dL), CKD–mineral bone disorder (calcium, phosphate, PTH, vitamin D), hyperkalemia, and metabolic acidosis.
Refer to nephrology for eGFR <30 (stage G4), severely increased albuminuria (A3, UACR >300 mg/g), rapid progression, or refractory complications.
Plan kidney replacement therapy ahead of crisis: consider transplant evaluation and dialysis access (arteriovenous fistula) when eGFR is ~15–20 mL/min/1.73 m² or the 2-year risk of kidney failure is high; the decision to start dialysis is driven by uremic symptoms, not GFR alone.
