27-year-old with a self-limited febrile illness — fever, sore throat, rash, and swollen lymph nodes lasting about two weeks — this is the classic acute retroviral syndrome of acute HIV infection, which 40–90% of newly infected people experience
Nonspecific "flu-like" presentation that resolved on its own — acute HIV is frequently mistaken for influenza, COVID-19, or mononucleosis, which is why it is commonly missed
Symptoms appearing then a positive test six months later — the timeline fits seroconversion, with anti-HIV antibodies developing during the first ~6 months (recent infection) after the acute viremic phase
Reactive HIV screening test followed by a confirmatory antibody differentiation assay — this mirrors the recommended CDC/lab testing algorithm and confirms established HIV infection
Symptom onset roughly 2–4 weeks after presumed exposure — acute HIV symptoms typically begin 2–4 weeks after infection

NOT infectious mononucleosis (EBV) — although mono causes fever, pharyngitis, and lymphadenopathy, Calvin's reactive HIV antigen/antibody screen with a positive confirmatory assay establishes HIV, and acute HIV should be specifically tested for when heterophile-negative mono is considered
NOT influenza or COVID-19 — these cause an overlapping acute syndrome but would not produce a reactive HIV screening immunoassay with a positive antibody differentiation test
NOT secondary syphilis — syphilis can cause fever, rash, and lymphadenopathy, but it is diagnosed by treponemal/nontreponemal serologies, not a reactive HIV antigen/antibody assay
NOT acute viral hepatitis — hepatitis presents with transaminitis and jaundice rather than the pharyngitis-rash-adenopathy retroviral syndrome, and is confirmed by hepatitis serologies
NOT a simple drug or viral exanthem — an isolated rash would not explain the constellation of fever, pharyngitis, and adenopathy, nor the confirmed positive HIV testing

Key Distinguishing Test

Initial testing uses a fourth-generation HIV-1/HIV-2 antigen/antibody combination immunoassay (detects p24 antigen plus antibodies); a reactive result reflexes to an HIV-1/HIV-2 antibody differentiation assay to confirm established infection.
When the differentiation assay is negative or indeterminate but acute HIV is suspected, an HIV-1 RNA (nucleic acid) test is performed — a detectable HIV RNA confirms acute infection, whereas an undetectable RNA indicates the initial screen was a false positive.

Board Note

Screen broadly: the USPSTF recommends one-time HIV screening for everyone aged 15–65 years, and clinicians should keep a low threshold to test for acute HIV in anyone with a mononucleosis- or influenza-like illness, since acute infection is a period of very high viral load and infectivity
Start ART in everyone with HIV as soon as possible after diagnosis (including same-day/rapid start), sending a genotypic resistance test before—but without delaying—the first dose
First-line initial regimens for most patients are a second-generation integrase inhibitor plus two NRTIs — bictegravir/tenofovir alafenamide/emtricitabine, or dolutegravir plus tenofovir/emtricitabine (or lamivudine) — or the two-drug regimen dolutegravir/lamivudine in eligible patients
Opportunistic infection prophylaxis is CD4-driven: start Pneumocystis jirovecii pneumonia prophylaxis (trimethoprim-sulfamethoxazole) at CD4 <200 cells/mm³, and add Toxoplasma prophylaxis at CD4 <100 cells/mm³ if Toxoplasma-seropositive; neither is indicated at Calvin's CD4 of ~800
Undetectable = Untransmittable (U=U): durable viral suppression eliminates sexual HIV transmission; offer PrEP to HIV-negative partners and other at-risk individuals
Monitor response with serial viral load (undetectable typically achieved in 8–24 weeks) and CD4 counts; failure to suppress by ~24 weeks should prompt an adherence and drug-interaction assessment
