Visual aura of zigzag/shimmering lights lasting about 30 minutes before the headache — a fully reversible visual aura evolving over 5–60 minutes and resolving before or as the headache begins is the classic aura of migraine with aura
Unilateral (left-sided) headache — unilateral location is one of the core ICHD-3 pain characteristics of migraine
Pounding/throbbing quality — pulsating pain is a defining migraine feature
Associated nausea — nausea/vomiting is a required associated symptom category for migraine

Why It's NOT the Similar Diseases

NOT tension-type headache — Beatrice has unilateral throbbing pain with aura, nausea, photophobia, and activity avoidance, whereas tension-type headache is typically bilateral, pressing/tightening, without aura, nausea, or disabling photophobia
NOT transient ischemic attack/stroke — her aura develops gradually over minutes with positive shimmering/zigzag phenomena that fully reverse and are followed by a typical headache, unlike the abrupt onset and persistent focal deficits of ischemia
NOT cluster headache (trigeminal autonomic cephalalgia) — she has no ipsilateral autonomic features (tearing, conjunctival injection, ptosis, nasal congestion) or short, strictly one-sided orbital attacks, and prefers to lie still rather than pace
NOT secondary/dangerous headache (e.g., subarachnoid hemorrhage, mass, meningitis) — she has a longstanding stereotyped pattern with no thunderclap onset, fever, neck stiffness, weight loss, progressive worsening, or new neurologic deficit (no SNNOOP10 red flags)
NOT medication overuse headache — her attacks respond to appropriately timed triptan use and she deliberately keeps triptan days low, rather than showing escalating headache frequency from frequent acute-medication use

Tension-type headache — ruled out by unilateral throbbing pain with aura, nausea, and photophobia
Transient ischemic attack — ruled out by gradual-onset, fully reversible positive visual aura followed by typical headache and a normal exam
Cluster headache — ruled out by absence of cranial autonomic features and the multi-hour, non-orbital, throbbing character
Secondary headache (hemorrhage, tumor, meningitis) — ruled out by longstanding stereotyped pattern with no red-flag features
Medication overuse headache — ruled out by controlled, infrequent acute medication use with preserved treatment response

Key Distinguishing Test

Migraine is a clinical diagnosis based on the ICHD-3 criteria; a characteristic history plus a normal neurologic examination between attacks confirms it, and no imaging or laboratory test is required.
Neuroimaging is reserved for atypical features or SNNOOP10 red flags (e.g., thunderclap onset, new deficit, fever, progressive or new headache after age 50).

Board Note

Classic profile: young to middle-aged woman with recurrent unilateral throbbing headache lasting 4–72 hours, plus nausea and/or photophobia and phonophobia; aura, when present, is a fully reversible focal deficit (most often visual) developing over 5–60 minutes
Acute treatment: NSAIDs are first-line for mild-to-moderate attacks and triptans for moderate-to-severe attacks; the combination of a triptan plus an NSAID has the greatest net benefit, and drugs work best taken early
Triptans/ergots are contraindicated with established cardiovascular disease, prior stroke, or uncontrolled hypertension; gepants and ditans (non-vasoconstrictive) are options when triptans fail or are contraindicated
Prevention is indicated for frequent or disabling attacks: first-line oral options include beta-blockers (propranolol, metoprolol), topiramate, valproate (avoid in people who can become pregnant), amitriptyline, and venlafaxine; CGRP antagonists are also effective
Avoid medication overuse headache by limiting acute treatment (roughly triptans/combination analgesics fewer than 10 days per month and simple analgesics fewer than 15 days); management centers on education, limiting/withdrawing the overused drug, and starting prevention
Refer for red flags, diagnostic uncertainty, refractory or chronic migraine, or when specialized preventives (CGRP agents, onabotulinumtoxinA for chronic migraine) are being considered
